If you want to surrender an animal, you must choose and check one of the following options or we will be unable to assist you:

The animal is not mine and I am unable/unwilling to take responsibility for it.

I am the owner of the animal and I am having a problem with it. The animal either needs more attention (time, attention, medical expenses, etc.) than I am able to or want to provide, or has behaviors that I can no longer tolerate.

I am the owner of the animal and my lifestyle has undergone some change that has made it inconvenient or impossible to continue to keep the animal. (This includes death, marriage/divorce, allergies, children, illness, moving and unable to take animal, travel now required for job, and similar reasons.)

Other reason for Surrender (Please describe):

Do we have your permission to forward this information to all interested parties?

Yes, you have permission to forward my information to all interested parties.
No, you do NOT have permission to forward my information to anyone.

Is there ANYTHING additional you would like us to know about the animal or situation that might be helpful, like how long you can or cannot keep the animal before it has to go, or, if you are the owner, exactly why you can no longer keep the animal?  Sometimes, we have alternatives to surrender we can suggest and/or help you with.

Is the animal in need of immediate or emergency medical care? (If so, and it is not your animal and you can afford to do so, you should take the animal to a veterinarian immediately.  If so, and it is your animal, short of destitution on your part, you should take the animal to a veterinarian immediately.)

Emergency medical care is needed.
Emergency medical care NOT needed.

Check all that apply:

I can help with vaccinations.
I can help with spaying/neutering.
I can keep the animal until a home/foster home is found.
I can help with boarding costs.
